Observation

Adriana Torres De Shulkind background check expired Sept. 2020. She resubmitted background forms in Oct. 2021 and Nov. 2021 but fingerprints were not completed so they were archived. When the sites monitor inspection was conducted on Feb. 2, 2022 the background check was pending. I was told by the director that Shulkind had a fingerprint appointment on Feb. 14, 2022. I reached out to the Merit background unit and was told that Shulkind had submitted a new application on Feb. 3, 2022. Payment was posted to her applicaiton on Feb. 8, 2022 and she was sent notification to do fingerprints. She did schedule fingerprints for Feb. 14, 2022 but she was turned away because her last name in her applicaiton was not correct. She didn't have her current legal last name listed in her Merit account. On Feb. 15, 2022 she reached out to Merit and corrected her last name. She was told her current application would be archived and that she would have to reapply with the correct last name and then a new OCA number would be issued for her to complete the fingerprint process. As of March 10, 2022 she has not submitted a new background application.
